Parental consent and waiver

I understand basketball is a physical sport involving inherent risks of injury. I voluntarily allow my child to participate and release Way of Elite Basketball Club, its coaches, staff, volunteers, and affiliated facilities from liability for injuries sustained during participation, except in cases of negligence.

In an emergency, I understand the club will make reasonable efforts to contact me. I grant permission for club staff to seek appropriate medical attention for my child if deemed necessary.
